We are a national collective of local, independent direct primary care practices providing care to employers seeking to invest in the health and wellbeing of their employees, businesses, and communities. Direct primary care or DPC is a membership model where personalized and comprehensive primary care services are provided for an affordable, monthly fee. Through trust and comfort from local practices, direct and immediate access to your doctor, and time and attention for personalized care, the model is built around patients and their families having a better experience and achieving optimal health. Through the 3x5 Health Collective, we support both starting and existing DPC practices across the country by connecting them with employers providing DPC to their employees, particularly those with multiple locations, more complex employer sponsored health plans or a distributed workforce. By partnering with 3x5 Health, businesses of all types and sizes are seeing a healthier, more satisfied team - at a significant savings compared to traditional healthcare options.
