A1 Service Coordination team members provide support with: Securing Initial enrollment onto the NHTD / TBI Medicaid Waiver program Maintaining Annual/On-going Enrollment on the NHTD / TBI Medicaid Waiver program Assisting with a Discharge back to a home of your choice in the community after short-term or long-term Nursing Home Placement Securing waiver and non-waiver supports and services that you need to prevent Nursing Home Placement (such as home care, home attendants, living skills training, behavioral support, medication management, nursing visits, rehabilitation services, and other community resources) Making referrals to waiver and non-waiver providers within the community for supports and services that will maintain your health, safety and well-being Maintaining benefits and entitlements (such as Medicaid, Medicare, Social Security, SNAP, etc.) Identifying your goals to increase your independence in your home and in the community Identifying your goals to ensure that you remain living in your home and prevent Nursing Home placement now and in the future
