Waiver services are provided based on the participant’s unique strengths, needs, choices and goals. The individual is the primary decision-maker and works in cooperation with providers to develop a plan for services. This process leads to personal empowerment, increased independence, greater community inclusion, self-reliance and meaningful and productive activities. The dignity of risk and right to fail are integral parts of the waiver’s philosophy. The philosophy of the waiver supports the participant’s right to choose where to live, who to live and socialize with, and what goals and activities to pursue. Waiver programs are specifically for people who would otherwise be in an institution (nursing home, hospital) for long-term care. The TBI Waiver promotes traumatic brain injury care and recovery in the least restrictive setting possible, such as a person's own home, or the home or a loved one. The NHTD waiver program is a home and community-based program that helps New York's Medicaid-eligible seniors and people with physical disabilities receive comprehensive services they need while they live in a community-based setting, rather than in a nursing home, congregate care setting, or other institution.
