About Active Community Health Center
Active Community Health Center was created on a foundation of strengthening our community one family at a time. We offer outpatient counseling services and programs throughout Broward County. Our clinicians will travel to your home, office or school top provide high quality behavioral health counseling accommodating your needs and schedule. Our Founder & CEO, Pablo Uriarte, worked tirelessly in the Broward County Public School System as a Social Worker. However he always felt more could be done to help families in need. He embarked on his own in 2015 and formed ACHC. In 2019 we opened 3 locations, received a coveted CARF Accreditation and now offer - Telehelth services.
