About Aviva Health
Aviva Health, formerly known as Umpqua Community Health Center, is a Federally Qualified Health Center (FQHC) designed to serve and assist people who have limited access to health care in southern Oregon. We live by our motto of "caring for the community on a personal level" everyday. Services include: Family medical and dental care including pediatrics, behavioral health on site, and access to special programs for uninsured and under-insured patients.
