Best Care is a physician-led, integrated chronic disease management solution that places primary care at the foundation of health system transformation. The Best Care model embeds a Certified Integrated Disease Management Clinician (CIC) in a primary care setting where the patient normally receives care. Working with the primary care provider the CIC delivers on all care quality guidelines and standards. This improves patient experience by providing high-quality care close to home, improves provider experience by delivering team-based care and alleviating pressure on primary care, specialty care and hospitals. Best Care’s work to date has had significant impact in Southwestern Ontario by avoiding 24,766 hospital bed days, replacing 22,000 primary care visits and saving more than $31 million in hospitalization costs.
