About Care Navigation Consulting

I founded CNC to be a consulting partner for providers and vendors to positively impact the delivery of care for our seniors throughout the care continuum. It was established to help you accomplish your goals through: Regulatory Training and Education Clinical Programming Assessment and Training; with a focus on Quality Measures Establishment of upstream and downstream partnerships, what to know, what to ask and what to provide. Clinical/Regulatory/Operational Assessment of current Rehabilitation Program; Including auditing and documentation training. I want to provide you SOLUTIONS!! My mission is to provide you with the information you need to effectively and efficiently navigate patients through the care continuum! As a Physical Therapist who has worked in the post-acute settings for over 20 years, I have a passion for our Seniors and an appreciation for the caregivers doing everything they can do to make the experience positive and successful. I believe it is possible to provide what the patient needs and do it in a way that allows providers to be sustainable. I have always lived by the statement, “if you do right by the patient, the rest will follow.” While many aspects of our industry have changed, I still believe in this statement and will work to make it happen with you and your patients or clients!
