About Care Partners
Care Partners is a community-based nonprofit hospice and palliative care provider that has been serving the Portland Metro area for over 40 years. Every day we deliver on our mission to provide exceptional care without exception. In order to do this, we offer: Personalized care - We listen and respond to individual needs. More health care - We offer focused care programs tailored to manage the four most common serious illness diagnoses: cancer, heart failure, chronic lung disease, and dementia. Psychosocial care - A life-limiting illness is one of the most stressful experiences that a person and their loved ones can face. We take the time to provide compassionate support and work to transform: Fear into acceptance, Grief into hope, and Pain into peace. Care for all members of our community - Our services are tailored according to the individuals that we serve. Additionally, we provide support to those without insurance coverage with medical costs, rent, utilities, and food as required. Care Partners has been voted one of the 100 Best Nonprofits to work for year after year.
