About Case Management Cymru
Case Management Cymru delivers person-centred specialist case management to adults and children with acquired brain injury, birth injuries, learning difficulties, spinal or orthopaedic injuries, mental health issues, including PTSD and older adults with diminished capacity. From our base in Rhondda Cynon Taff (RCT) we provide services to individuals in South and West Wales and beyond. At CMC we understand Welsh health and social care services and can adapt to the variety of socio-economic communities that exist across the region. We also draw upon a wide local network of professionals and services, enabling us to work effectively and collaboratively across sectors. We pride ourselves on providing bespoke services, recognising the uniqueness of every client; each having very different aspirations and challenges. We are responsive and flexible; encouraging creative thinking to help the client achieve their goals, which they have been involved in setting. CMC recognises that the Case Manager’s ability to build rapport and trust with a client and their family is as important as having the right experience and expertise. Alongside our Case Management Service, we are registered with Care Inspectorate Wales (CIW) to recruit and manage hand-picked, private care teams on behalf of our clients, allowing us to provide a fully holistic, individualised service.
