About Central Oregon Health Quality Alliance
COHQA's mission is to cultivate an environment where independent practices develop and share ideas to improve the care of patients. It began in 2014 as a program through our sister organization, the Central Oregon Independent Practice Association (COIPA). Over the last 7 years, COHQA has awarded $1.5 million in grants to 56 projects ranging from improving diabetes care to providing free foot clinics to patients experiencing homelessness to funding training and support for caregivers of chronic pain and addiction patients. Grantmaking Priorities ● Provider-Led Health Care Innovation: Goal to implement intervention(s) specific to health care delivery systems, clinicians, and/or patients that focus on the Quadruple Aim of: Improved Quality, Reduced Cost, Improved Patient Experience, and Improved Provider and Staff Experience in the communities that COHQA serves. ● Community-Led Health Care Innovation: Innovations to improve community health, health care, and patient or provider experience can and should come from the community itself. We encourage community members, as consumers of health care, to partner with health care providers to develop and implement fresh ideas to improve care delivery. ● Vulnerable or underserved populations: COHQA would like to improve health and care for the most vulnerable or underserved members of our community. We support projects that address disparities in care due to socioeconomic, racial, or special needs (chronic diseases, disabilities, etc.). ● Clinical Integration and Multi-Sector Coordination: Implementation of intervention(s) that advance clinical and financial coordination and integration of stakeholders in the communities we serve. ● Skill Transfer: Efficiently and effectively to transfer skills and learned processes to other providers, clinics, or community organizations in a collaborative fashion.
