Chorus Community Health Plans, formerly Children’s Community Health Plan, is a health insurance organization offering both BadgerCare Plus and Individual and Family Plans and services designed to meet the needs of adults, children and families. We customize our services for our members and help empower them to take charge of their health and wellness while also bringing together community partners and providers to reduce health disparities, improve health outcomes, and help build stronger communities.
