About CIP Rehabilitation

Mission Statement: Community, Independence & Performance are the three pillars in which CIP Rehab is built. We plan to provide physical, occupational and speech therapy using evidence based therapeutic interventions to maximize patient performance within their community, helping them attain or return to independence. CIP Rehabilitation offers evidence based physical, occupational and speech therapy services to all individuals in a proactive approach to maximize a patient’s functional ability in their home or community. Therapy can be provided to all patients, regardless of “homebound status”. We consist of a team of highly skilled clinicians using advanced outpatient therapy techniques to achieve maximum therapeutic potential and patient driven goals. Therapists will work closely with the patient to create specific and customized plan of care that best suits their needs and desired goals. At the same time, we work as an inter-disciplinary team with other therapists, doctors, the patient, the family and the support staff at the facility to provide the best possible care. No matter the goals, living environment, or patient status we strive to increase patient performance. By working alongside staff at Assisted Living Facilities (ALFs), Personal Care Homes, Senior living communities, and families we hope to combat functional decline before it starts.
