Colonial Poplin has earned the reputation of being one of New England's finest skilled nursing facilities. Through our ongoing quality improvement process, we continuously review the integrity of our family owned and operated business. Our values endorse our belief that the people we care for are our highest priority. * Colonial Poplin is a skilled rehab facility that can support all of your respiratory needs. Our goal is to provide short term interdisciplinary rehabilitation for patients with a primary pulmonary or cardiac diagnosis and with acute conditions affecting their functional abilities. Patients are taught self-management strategies enabling them to reach their highest functional independence and prevent re-hospitalizations, often exceeding their prior level of function in a safe and effective manner. In addition to our comprehensive rehab, our cardio pulmonary program includes: Medical Director and Nurse Practitioner visits 4-5 times per week. On-sire Respiratory Therapist performing weekly rounds, Extensive resident and family education, Individualized treatment plans, Oxygen Therapy, C-PAP/BI-PAP, Aerosolized medication management, Registered Dietician, Secretion Mobilization & Airway Clearance, Chest tube care, Diagnostic testing. * Other services include: Physical Therapy, Occupational Therapy, Speech Therapy, and Advanced Wound Care Management including wound vac, Treatment of Joint replacements, Traditional skilled nursing care, Hospice Care, Sub-acute services including IV Therapy, Geriatric mental health services, Full activity program and bus trips, Long term and respite care, Full service cafe on-site, and Poplin Way Assisted Living Community.
