At Comfort Homesake, our unwavering commitment centers on supporting underrepresented communities by providing essential services related to palliative care, end-of-life, and hospice services. We are dedicated to offering training, education, and advocacy to ensure our community is well-informed about advanced directives and end-of-life planning. In 2018, our Result Accountability Report was a testament to our dedication to end-of-life support. We provided vital in-service training at various institutions, empowering hundreds with essential skills. Our endeavors have garnered accolades from both the Oakland Mayor's Office and the Alameda County Board of Supervisors, who bestowed us with official Proclamations in recognition of our contributions. Our dedication extends beyond training; we're equally devoted to supporting those in need. As partners in the No One Dies Alone program, we extend solace and companionship to individuals in their final stages of life, assuring them someone genuinely cares about them and that they will not face their last moments in solitude. We actively participate in the Friendly Visitor program, providing meaningful fellowship and comfort to Highland and San Leandro Hospital patients to alleviate feelings of isolation and loneliness. We emphasize helping diverse seniors, medically fragile, those with disabilities, and homebound individuals. In our commitment to inclusivity, we partnered with the Alameda Contra Costa County Medical Association to develop community ambassadors from diverse backgrounds and translate advanced healthcare planning resources into Chinese, Spanish, and Korean languages. While we strive to promote nationwide awareness of advance care planning and hospice utilization through education, consultation, and advocacy, our daily focus remains on our local underserved communities. We are committed to delivering compassionate end-of-life support services that actively contribute to the well-being of our communities.
