About Community Health Center of West Palm Beach
The Community Health Center exists to share the love of God by providing non-emergency medical, and eye care to those who do not have access to health care services, and assist them in establishing a continuum of care through conventional health care systems to promote a healthy body, mind and spirit. The Community Health Center mission is to ensure access to a comprehensive health care system and the delivery of quality services for the residents of Palm Beach County. The clinic is located at 2100 W. 45th Street, in West Palm Beach, Florida. That is the southwest corner at the intersection of 45th Street and North Congress Avenue. Medical Clinic Medical care for all ages is provided free of cost to those who qualify. No appointment is necessary to see a physician. Just Walk-In to our clinic during our regular hours of operation. Vision Clinic Our optometrists provide comprehensive eye exams and prescriptions for eyeglasses. They evaluate and treat many eye problems including infections, minor trauma, inflammations, glaucoma, and diabetic eye disease, using state of the art instruments and techniques. Chronic Disease Management Clinic Treating high blood pressure can take a multi-pronged approach including diet changes, medication, and exercise. Diabetes management requires awareness. Know what makes your blood sugar level rise and fall — and how to control these day-to-day factors. Diabetes is a lifelong, total-body health condition. We can help you manage your disorder, and your overall health, effectively. The Community Health Center seeks to not only meet your physical needs but also your spiritual needs. We have a chaplain on staff that prays and ministers to every patient that wishes to receive the word of the Lord. All prayers and counseling is done on a voluntary basis.
