About Community Health Council of Wyandotte County
The mission of the Community Health Council of Wyandotte County is to enhance health outcomes in Wyandotte County through informing, collaborating, aligning, mobilizing and activating organizations, government entities and community members. We believe that residents have the ability, and an inherent right, to provide leadership in the shaping of physical spaces and public policies which impact their health. CHC’s responsibility is to ensure the collective experiences and expertise of Wyandotte County residents are represented in matters of health design and policy, as well as identify and maximize opportunities for collaboration, planning and implementation of effective community health improvement initiatives. CHC is a non-profit, 501 (c) 3 community health collaborative of hospitals, safety net clinics, federally-qualified health clinics (FQHCs), mental health providers, public health departments, academic research institutions and health care funding organizations. CHC member institutions support the work of the organization on a pre-determined pledge that is renewed each fiscal year. The organization was formed in 2003 by executives from local hospitals, safety net clinics, and public health foundations as a response to findings published in the Mayor’s Access to Healthcare Commission Report. Committed to the foundational purpose of improving access to healthcare for the uninsured and medically underserved, CHC initially focused on issues related to inter-clinic communications and access to specialty care. In its first year of existence, CHC was able to get several Wyandotte County safety net clinics set up on KC Care Link, a coordinated care platform allowing them to communicate with each other in regards to shared patients.
