About Community Healthcare of Texas
Community Healthcare of Texas understands that choosing end-of-life care and support is a difficult decision. Introducing end-of-life care does not mean you are giving up, but that you are giving your loved one the gift of the expert care and support that is needed at this time. Choosing end-of-life care means you want to enhance the quality of your time together. Community Healthcare of Texas has been providing hospice home care, inpatient care, and respite care since 1996 to patients that have been diagnosed with a life-limiting illness and given a prognosis of six months or less. All care and support are provided wherever our patients call home - a personal residence, assisted living facility, nursing home, hospital, or one of our three home-like hospice inpatient facilities. Our Hospice Interdisciplinary Team of physicians, nurses, social workers, chaplains, and nurse aides work closely to provide the best plan of care to meet the needs of our patients and families. Community Healthcare of Texas also provides perinatal and pediatric hospice and palliative care programs for those with a serious illness that are not ready hospice. As the largest not-for-profit hospice and palliative care organization in the nation, Community Healthcare of Texas has committed to increasing access to hospice and palliative care, providing community education and outreach, offering special programs enhancing quality of life, and activism in local, state, and national healthcare and hospice organizations.
