Continuum has developed a reputations for fast response times, same day or 24-hour admissions, and 5 days per week hospice aide visits as requested. At Continuum Care, we strive to make the most of now and optimize the time given us by our patients and families. Your goal is our goal and we care for your loved ones as if they were our own. The staff at Continuum are trained professionals using their decades of experience to provide the best care possible, answer all of your questions, and alleviate any concerns you may have. Calls are answered 24 hours a day by one of our Registered Nurses or triage team members who can dispatch an RN to your location. Your comfort and peace of mind is our priority.
