"Our Heart is in Your Home" Many Seniors can remain safely at home. Medicare pays 100% for Home Health from your Medicare Part A benefit. Free evaluations are give in the privacy of your home to determine eligability. Our Nurses can monitor and assess changing medical conditions. They also teach you about your medications, disease process and tips to keep you home and out of the Hospital or Skill Nursing Facility. This education is also useful to the families of our Seniors. Therapists help you to be safer in your home by evaluating safety hazards, such as in your bathroom, stairs etc. Therapy can help you to prevent a fall that could land you in the hospital or skilled nursing facility. Therapy is also great in the first few weeks after joint replacement, to help you get back on your feet and to manuever safely doing those things that were not so difficult before surgury. Home Health Aides can help to assist you in those times when you need help to do those activities of daily living such as bathing when assistance in needed after surgery or general weakness. Social Workers are available to assist and support our patients and family. They also assist you to make decisions about community resource.
