About EldersChoice of Connecticut
EldersChoice of Connecticut, LLC is a 24/7 Non-Medical Homemaker Companion Agency registered in the State of Connecticut, registration number HCA0000489. As a Homemaker Companion Agency, EldersChoice screens and refers trained caregivers to provide activities of daily living or specialized care in an individual’s residence or independent living environment. EldersChoice provides ongoing non-medical care planning and comprehensive case management services for clients and their families. EldersChoice has operated in Pennsylvania since 2006, and in Maryland since 2009. EldersChoice is not a medical provider. Our company refers experienced and trained caregivers to provide non-medical live-in care services 24 hours a day, seven days a week. Our services enable individuals to remain independent and out of restrictive care settings. The caregiver assists the client with a variety of needs, including personal hygiene, grooming, meal preparation, medicine reminders, housekeeping and laundry services, transport or accompany the client to medical appointments, help or perform errands for client and monitor, supervise and assure the well-being of the individual. Every case is different and we provide a wide range of non-medical care services as needed. One primary caregiver lives continuously with the client for one to three months before taking a break. So there is no lapse in quality or continuity of coverage, EldersChoice provides a replacement caregiver until the primary caregiver returns. Before we refer any caregiver, EldersChoice staff conducts a complete case assessment at no cost. This assessment could be at a private home, a nursing home, rehabilitation center, hospital, or independent living facility. EldersChoice supports and actively monitors the client/caregiver relationship to assure that the established service plans and care goals are being met.
