About Elements Support Services
Elements Support Services is a specialist rehabilitation service meeting the needs of children, youth and adults with a brain injury or other serious trauma. Working in coordination with the Rehabilitation Team our support is instrumental and focused on the individual reaching their full potential. Rehabilitation Support Community Based Services At Elements Support Services we are committed to ensuring that our clients receive individualized community based services that will enable the individual to attain a preferred quality of life We work under the direction of the individual’s Moving On Moving On Services Following rehabilitation efforts designed to take advantage of the active recovery period, people with ABI and their loved ones often need to reinvent themselves as a result of their experiences. People need to feel empowered and able Social Work Elements provides family support, counselling and consultation social work services. We believe that every client and their family is entitled to emotional, psychological and practical support following an acquired brain injury or diagnosis of a neurological condition. Elements community based social Referrals can be initiated by calling our office: Call (905) 393-5724 and speak with one of our Clinical Service Coordinators. Our Clinical Service Coordinators are behavioural specialists (often behavioural therapists by training) with extensive theoretical and practical knowledge. We will review your current needs and make arrangements to meet with you to discuss how we can be helpful. We will likely ask you to ‘pull together’ any additional relevant information for review.
