Elevate Mental Health Supports is a community-centered program based in Boulder, CO, designed to serve individuals ages 12+ living with serious mental health challenges. We specialize in psychosocial rehabilitation, combining therapeutic coaching, case management, and independent living skills training to promote independence, self-awareness, and long-term stability. Our services include: • Mental Health Consulting • Rehabilitative Case Management • Independent Living Skills Training • Therapeutic Coaching grounded in CBT • Support Groups for peer connection and empowerment We work with clients navigating conditions such as psychosis, bipolar disorder, and dual diagnoses. Services are available in-person and virtually to meet a range of needs. Led by Leonora Otto, MAP, our approach integrates cognitive behavioral therapy, dialectical behavior therapy, Adlerian theory, and nature-based modalities. Leonora brings 10+ years of experience across inpatient, residential, and outpatient settings, with a deep commitment to holistic, accessible, client-centered care. Through nature-based initiatives, including bimonthly donation-based nature therapy events, Elevate fosters healing through connection—with the self, the community, and the earth.
