Elm York is an Adult Care Facility certified as an Assisted Living Program with a Licensed Home Care Service Agency. Through these specialized services, residents receive assistance with Activities of Daily Living and skilled services. This allows each resident of Elm York within a social model to age in place and feel a sense of self-dignity. Our program provides an alternative for the elderly living alone without needed care. Services include care management, counseling, advocacy, activities, as well as residential services. Our aim is to provide a warm, safe environment while encouraging independence. Our social living environment is an alternative to institutionalized care and to the nursing home. Our residence is located in a residential neighborhood of East Elmhurst, New York. The facility is modern, fire proof, seven story building. All rooms have private bath, with a telephone on which to receive incoming calls. We provide three full meals daily. Snacks are available two times daily. We also provide laundry and housekeeping services. At Elm York residents are cared for by physicians affiliated with some of the finest hospitals in the area. These doctors include Internists, Psychiatrist, Optician, Podiatrist and Dentist. The physicians visit the home on a regular basis. Lab and Diagnostic services include blood tests, EKG and X-Rays which are performed on site by exemplary service care givers. Residents can receive nurse services available seven days a week. Our devoted nurses provide care for those residents who are in need of same, such as Insulin Therapy and Wound Care. In addition to the aforementioned services, a Home Health Aid trained to assist the residents in need of intermediate supervision on their Activities of Daily Living Skills can be provided for.
