Encore Senior Care Network is a Limited Liability Company, established in 2013 and serving the Dallas/Fort Worth metroplex and surrounding areas. Our Mission: “Advocacy and Excellence in Senior Care”. Our Vision: Through a network of providers and vendors, deliver appropriate non-medical and medical care for seniors and their families in the setting of their choice with integrity and achievable outcomes. Our Objective: Develop and implement the architectural infrastructure and blend partnerships among providers and vendors to meet the longitudinal transitional needs of our clients. Our pre-acute care focuses on Wellness and Case Navigation; our post-acute services focus on care due to an illness, trauma, surgery or secondary conditions. The Encore Wellness Program is designed to offer clients access to healthcare and community resources in the setting of their choice. It is our hope to enhance the quality of living for clients and allow them to enjoy and maintain the optimum level of independent living. Encore is set to launch a customer service center designed with a toll-free number where live agents will address questions in both English and Spanish and assist in providing requested services. We are also getting ready to go live with our website, which will serve as an interactive virtual venue for clients to read articles on healthcare, schedule appointments with their provider, and view calendars of activities in the community.
