Evergreen Health Care is a non-profit organization that operates four new primary care health centers in Maryland. Evergreen Health Care was founded in 2013 by Peter L. Beilenson, the former Health Commissioner of Baltimore City, whose desire is to improve access to and quality of primary care in Maryland. Four new health centers offer Marylanders a robust patient-centered health care experience. The health centers are staffed Family Medicine Physicians and Nurse Practitioners, Care Coordinators, Health Coaches, and Medical Assistants to address your mental, physical, and social health needs. Evergreen Health Care is committed to providing patients of all ages with excellence in primary care through a team-based, patient-centered model. Patients can expect to receive more robust and personalized service than the typical primary care setting. Features include longer visits with your Provider, customized health care agenda setting, access to Health Coaches who are licensed mental health professionals, and high-touch Care Coordination services.
