About Functional Life Achievement
Functional Life Achievement, Inc. (FLA) is a not-for-profit Early Intervention agency approved by New York State Department of Health. FLA provides monolingual and bilingual early intervention home based services and evaluations in all five boroughs. The Early Intervention Program: The Early Intervention Program (EIP) is a public program for infants and children under the age of three who are either suspected of having or at risked for developmental delays or disabilities. All potentially eligible children must be referred to their appropriate county program. EIP is funded by New York State and county governments. All services provided under EIP are at no out-of-pocket cost to parent. Health insurance is not required, however when applicable, it may be used to cover some cost. A child’s eligibility for EIP can be determined only by state-approved evaluators and all services must be authorized by the county. If a child is found eligible, the Service Coordinator, child’s evaluator(s), Early Intervention Official, and parents will collaborate to develop a plan for services—authorized by the county—that meets the specific needs of the child and family. Services are provided in locations most convenient for family, which includes home, daycare, or other community settings. If services are provided in locations other than home that require a fee, parents are responsible for paying the cost. EIP evaluations and services include, but not limited to, the following: • Speech Language Therapy • Occupational Therapy • Physical Therapy • Nutrition Therapy • Social Work and Family Training • Special Instruction • Autism/Behavioral Management Services • Vision Services • Service Coordination Services • Other
