About Health Action Together

WHO: We are a public-private partnership of grass-roots community and institutional leaders supporting the alignment of efforts and coordination of resources to address inequities in Sonoma County. WHY: We envision a Sonoma County where a person's skin color, zip code, or place of birth does not determine their health, education, or income. We plan to achieve this through centering the communities most impacted by structural inequities and democratic structures to set agendas, influence who makes decisions, and through cultivating ongoing relationships of mutual accountability with decision makers that change systems can we achieve health equity. HOW: By centering communities often excluded or deemed invisible by our social systems. We develop and foster relationship building by ensuring that there is representation of communities, often excluded or deemed invisible, at the decision-making table and in leadership roles and by listening, acknowledging wrongdoing, healing through powerful & sometimes uncomfortable conversations, taking action, and holding each other accountable to achieve trust and create significant change. WHAT: Supporting a racial equity co-designed process with community members of color and other marginalized populations most impacted by inequities we call the Agenda for Action (AFA), or a roadmap for change. The AFA is informed by the Portrait of Sonoma County 2021 Update (POS) report findings. And specifically, the AFA will be guided by communities not represented in the data like Black, Indigenous, people of color (BIPOC), LGTBQI, undocumented, BIPOC elder & youth and under-resourced communities, to name a few. We are also engaging with partner institution leaders in local government, non-profits, education, hospitals, FQHC’s, and other social sector institutions to learn about equity efforts they are leading to support their capacity to address inequities in our community.
