About Health Horizons International

Health Horizons International (HHI) is a non-profit organization dedicated to improving community health and expanding access to quality primary health care in the Dominican Republic. Our model is based on three pillars: - Developing Community Leaders — Our Community Health Workers Program trains and supports local individuals as partners in primary care delivery, focusing on chronic disease management and prevention. - Strengthening the Health System — We collaborate with health sector leaders to build capacity for achieving public health priorities, including addressing the growing challenges of chronic disease through a primary care lens. - Improving Access to Quality Care — Our Chronic Care Program provides medication and counseling to patients in rural communities by creating a network of physicians and CHWs who provide continuity of care. Since 2009, HHI has grown from serving four communities, to impacting hospital programs across the Puerto Plata province and even influencing policy at the national level. HHI is one of the few international NGOs in the Dominican Republic that focus on the growing global epidemic of chronic disease, finding replicable and sustainable solutions that can be scaled across the nation. Our Vision One day, all communities will have the power to promote health and well-being, and to access quality health care as a basic human right. Our vision is for all people to have the opportunities of a healthy life, and to overcome the inequalities in health that disproportionately affect people facing poverty.
