About HealthShare Exchange
HealthShare Exchange is a regional healthcare data aggregator, offering services in health information exchange (HIE), population health data, and health data innovation. In collaborative spirit, competing health plans and hospital/health systems in the Delaware Valley formed HSX to enable electronic exchange of patient information with the goal of improving patient outcomes and managing and lowering healthcare costs in the region. Incorporated as a non-profit organization in May 2012, HSX has a membership in 2019 that includes more than 170 independent ambulatory practices, seven behavioral health organizations, six accountable care organizations (ACOs), more than 85 skilled nursing/long-term care facilities, a clinically integrated network, a nutrition assistance organization and more. • HSX helps to enhance care quality and efficiency by making patient medical record information securely and confidentially available to authorized providers and other care team members. Its HIE services address challenges in care transitions for patients and help to ensure that providers are better informed –– and that members can optimally align with new value-based care models. • HSX’s Population Health Program offers analysis of a vast repository of patient information and provides customized reporting –– and features its GRID (Gather, Report, Identify, Determine) data portal and its CPC+ data processing role. • HSX’s Innovation, Research, and Development arm provides an opportunity and process for start-ups and existing enterprises to build products and services for consumers and businesses based on access to a large, growing, and evolving clinical data set –– and features its MarketStreet platform for creation of new health data applications.
