About Healthy Community Alliance
The Healthy Community Alliance is a rural health network chartered in 1996 by key community leaders concerned about maintaining and improving local health care services during a time of great change. Our service area encompasses portions of four Western New York counties along Route 39 – Cattaraugus, Chautauqua, Erie, and Wyoming. Within that area are some 3,100 Native Americans residing on the Cattaraugus Indian Reservation; 200 Old Order Amish; 16 school districts; and four NYS prisons housing more than 7,000 inmates. Some of our programming includes: • Navigator Program - HCA is a designated Navigator agency for the NY State of Health, a healthcare marketplace. They help get qualified individuals free or low cost health insurance. • Patient Activation Measure (PAM) - A program designed to target Medicaid populations and help them take advantage of preventative medical care, where they may not have been doing so. • Partner with Springville Concord Elder network (SCENe) to establish a “one-stop” for senior services • Falls Prevention - Education, programs, and activities throughout Cattaraugus County. • Safe Routes to School Initiative- Focuses on the importance of providing safe routes for children to walk and bike to school.
