HN is an award-winning company that uses AI to predict and prevent care need. We provide practical applications of population health management, going beyond simply identifying high-need patients to proactively intervening and supporting them to improve their health outcomes. Established by clinicians and researchers at Sweden’s Karolinska Medical Institute, our journey in the UK began in 2015.
