Home Physicians Group has a team of physicians and nurse practitioners dedicated to providing the highest quality in care in Skilled Nursing, Assisted Living, Independent Living and in the comfort of a person’s home. Through our various managed care services, we provide cost effective specialized care in the Orlando and Central Florida area. From direct patient care for the chronically ill, to transitional care programs from hospital to home. Our mission is to provide high quality healthcare to individuals in their places of residence in the Orlando and Central Florida community. Home Physicians Group medical providers visit nursing homes, assisted living facilities, provide in-home appointments for home bound patients and provide hospice care. In addition, our medical providers will order lab tests, xrays, EKG’s, and whatever “ancillary” services or procedures that may be required to fully evaluate any medical problem — and you almost never have to leave your home for these to be done! Furthermore, we also work with several home health care agencies and can help provide medical supplies, physical therapy, nursing, wound care and hospice care at home. Home Physicians Group medical practitioners are trained to work closely with Alzheimer’s and Dementia patients and their families. Equally important, we believe in open communication with families and their loved one, which results in higher quality care for the senior.
