By working in partnership with hospitals, consultants, community providers and GPs to provide ‘out of hospital’ care we can prevent some people being admitted to hospital and enable others to come home more quickly. Our highly skilled multi-disciplinary nursing and therapeutic teams can safely provide holistic care in the patient’s home, seven days a week. We use remote patient monitoring technologies. Our services: • increase hospital capacity • improve patient flow • reduce delayed discharges • deliver great patient outcomes We work through the following clinical pathways delivered by nurses, therapists and support staff: • virtual wards • early supported discharge • discharge to assess (D2A) • reablement • admission avoidance • bridging packages of care The features of our work across these pathways include: • working in partnership with other providers • pre-discharge assessment • facilitating same day transfer/discharge • home based care up to QDS (four times a day), seven days a week • a 24/7 on call service, every day of the year • Real time activity data / analytics • KPI’s and business intelligence reporting.
