Horizon Integrated is a unique, specialist integrated rehabilitation and recovery residential service for males and females with mental health and/or intellectual disabilities. This service caters to individuals who may have behaviours that challenge alongside additional social and complex needs, including those who may have a forensic history. Horizon House offers a smooth and structural transition from a range of health services back into the community, where participation in both social and domestic activities is vital for their quality of life in the community. At the heart of the service are clinicians Sean O’Hanrahan MA BA (Hons) RMN and Gordon Shannon RMN (Distinction). With over 25 years experience at both managerial and clinical level, Sean O’Hanrahan is the Registered Manager of Horizon Integrated. Sean is a Registered Mental Health Nurse (RMN). His expertise is around managing residential specialist services, rehabilitation hospitals, supported accommodation and has significant clinical experience in forensic, acute and community step down mental health settings at a national level. He is passionate about the delivery of high-quality support for clients that optimises their opportunity for full recovery and reintegration back into their local community. Alongside him is Gordon Shannon, who is the Clinical Director of Horizon Integrated. Gordon is a Registered Mental Health Nurse (RMN) with over 16 years managerial and clinical experience in locked rehabilitation hospitals. Gordon’s expertise is based around the assessment and management of individuals with complex mental health conditions. He has experience in both adult and forensic mental health settings delivering intensive input to help break the cycle of repeat hospital admissions and allow clients to live more independently in a community environment.
