Kalos Health is a New York State-authorized Managed Long-Term Care (MLTC) plan that helps individuals 18 and older with a chronic illness and/or health disability who live in any one of the following six Western New York Counties: Erie, Niagara, Genesee, Orleans, Chautauqua and Monroe. We work closely with the member, their physician(s), and their family and caregivers to give the health services and personal care needed. Our goal is to help individuals remain in their own home with assistance from our personalized care management and network of care providers.
