Established in 2015 by Liz Dunleavy, MSW, LSW, CMC (a licensed social worker and certified Geriatric Care Manager), Kith Elder Care realizes that the changes associated with aging can be difficult and that deciding on the best plan of action for yourself or an elder can be a daunting task filled with questions and anxiety. Kith Elder Care provides an extensive list of concierge services that are customized to the specific needs of each client. The first step is our detailed assessment process that provides a holistic overview of the client and their situation. We then create a plan of care that encompasses multiple facets of the client's life, and from there the level of ongoing care management services is determined that provides oversight of the client and any services they may be receiving. Through ongoing care management, changes in status are more readily identified and addressed.
