About My health for life

Funded by the Queensland Government through Health and Wellbeing Queensland, My health for life supports adult Queenslanders to create healthier habits with free, qualified Health Coaches, online support and more. My health for life is an evidence-based, behaviour change initiative, based on the HAPA model (Health Action Process Approach). It is a practical extension of the advice given by Health Professionals to their patients and clients that allows participants to better understand their health risks and supports them to achieve their health-related goals. Delivered in 6 or more coaching sessions, at a relaxed pace, over 14 – 18 weeks the initiative is designed to support individuals to discover ways of changing their day-to-day behaviour - they can live with. It’s provided in a variety of ways, from groups to phone or online coaching — locally, at home or at work — with culturally adapted versions. Primary Healthcare Professionals play a critical role in the community for type 2 diabetes, heart disease and stroke prevention, as patients turn to health professionals as the source of guidance.
