About Neighborhood HealthSource

Neighborhood HealthSource provides safety net health care on a sliding fee scale to a high-risk, hard-to-reach population that experiences some of the worst health disparities in the state of Minnesota. A Federally Qualified Health Center with locations in North and Northeast Minneapolis and Anoka County, our mission is to improve the health of our communities by providing quality health care services that are affordable and accessible, while advancing health equity for all. The impetus for our founding as a volunteer-run free clinic 50 years ago, and the goal that continues to drive our operations today, is the elimination of health disparities through the provision of culturally competent, high quality, affordable and accessible primary health care. We are a non-profit, safety net health care provider offering a full range of primary and preventative medicine for adults and children, including well-woman and well-child exams, immunizations, chronic disease screening and management, minor procedures, and comprehensive referrals. We believe that comprehensive primary and preventative care are rooted in patient engagement and education. Our team of community health educators present at health fairs, community events and area schools on a wide range of topics including breast cancer screening and breast health, diabetes prevention, blood pressure, colorectal cancer and men’s health, sexually transmitted infection testing and prevention, birth control access and healthy relationship counseling, and more. To deliver our work with as much impact and efficacy as possible, we collaborate with a wide range of government agencies and departments, nonprofit organizations, schools, foundations and area businesses.
