New Horizons Home Health Services was founded in 1995, with the vision of utilizing Christian values in providing home health care. Our vision is to provide care with commitment, compassion, and integrity not only to the population that we serve but to the staff of New Horizons as well. The dedicated staff are committed to assisting those in need. We proudly serve the members of our community who are disabled, chronically ill, or physically/cognitively impaired, pediatrics though geriatrics, who may need assistance, to remain in their own home or temporary residence as a viable option to health facilities or institutions. We focus our energy and passion into the continually improving to meet and exceed the expectations of the clients we serve. Our philosophy is not only to meet the physical needs of the clients entrusted to our agency but to create a place where the highest integrity, honor, and respect is paramount. We strongly believe in education about the options available, even before the need arises. At New Horizons we utilize a holistic approach in our application of home health care. Each client, as an individual with multifaceted needs, receives coordination of care through the RN case managers. Our nursing team collaborates with adult daycare, inpatient respite, outpatient therapy providers, ambulatory surgical centers, wound clinics, primary and specialty physicians, labs and pharmacies, as well as medical equipment providers to ensure that all aspects of the client’s needs are met. Recognized in 2007, as one of the top agencies in Indiana with the lowest incidence of emergent care visits. Through regular comprehensive assessments, RN’s recognize the need for proactive interventions and communicating information with primary physicians, resulting in new orders being obtained and quickly implemented.
