About Northern Counties Health Care
Northern Counties Health Care provides a full spectrum of quality, affordable, and compassionate health care services to patients of all ages, throughout the three county region of northeastern Vermont, known as the Northeast Kingdom. NCHC encompasses six community health centers - including a walk-in primary location, three dental centers, , and a Medicare-certified home care and hospice division. The community health centers are NCQA recognized as patient-centered medical homes. NCHC is a nonprofit corporation, established in 1976 by community residents concerned about their poor access to health care and has a rich history and tradition of innovation in caring for our friends and neighbors.
