Our interdisciplinary team approach is a unique model of care that allows our staff to personally get to know participants and their caregivers. Our team meets on a daily basis to proactively communicate and coordinate holistic care to prevent avoidable emergency visits, hospitalizations and nursing home placements, thus providing higher quality care.
