Patient Centric Solutions enables the secure, patient-directed sharing of health data between providers and patients through PatientShare, which is a combination of standards infrastructure and web-based applications. Using PatientShare, individuals set preferences that govern how their health data is shared and who—providers, family members, friends—are entitled to see it. PatientShare includes a transparent patient-mediated consent system and authorization server. Supporting both organizational and patient-directed policy, PatientShare is structured to easily integrate with any healthcare system or use case. PatientShare provides a means for the patient data to follow the patient, critical for addressing patients during transitions of care. The application allows the patient to select and share health-related records and clinical data with whom they want - when they want, enabling: • Seamless interoperability across health care systems • The highest level of identity, authorization and security standards • Personal control and transparency PatientShare integrates with 3rd party identity systems to verify that each of the exchange partners are who they say they are, using the NIST AAL2 and IAL2 standards. PatientShare is based on the FHIR API, the ONC recommended health care exchange standard. It also uses OAuth2, OpenID Connect, User-Managed Access, and HEART profiles; it is one of the few applications that incorporates all of these standards to address patient-mediated health data exchange. With Patient Centric Solutions, we can improve patient outcomes, quality of care, and efficiencies while reducing patient risk and healthcare costs.
