About Prescribing Care Direct

Prescribing Care Direct (PCD) aims to develop and deliver a high quality medicines optimisation and management service to GP surgeries nationwide. At PCD, we provide a bespoke medicines management service. Through our highly skilled and experienced pharmacist support team, our practice pharmacist model supports surgeries with a cost effective resolution to manage medicines related activities. PCD has recognised an increasing need for the deployment of practice pharmacist services. We provide a range of primary care medicines optimisation services, including care home and discharge support services. Our pharmacists are now also supporting practices with patient facing activities such as multiple morbidity and medication review clinics. NHS England has already highlighted that GPs spend a significant amount of time managing prescribing related issues, for example; reconciliation of letters, discharge forms, repeat prescription re-authorisations, medication reviews and acute prescription requests. The need for practice pharmacists is becoming common and more widely recognised to ensure the above tasks are managed efficiently. At PCD we have over 10 year’s of experience working in primary care. Our highly trained clinical pharmacists can deliver these time consuming matters in a more cost effective manner. Therefore releasing GP time, which is indispensable.
