We work closley with our patients/families and their physicans to provide continuity of care which results in the best possible outcome to get you back to independance in the comfort of your own home. We offer free in home evaluations by a Registered Nurse who can help families make informed decisions about their care. Our services include Skilled Nursing, Physical Therapy, Occupational Therapy, Medical Social Services, and Home Health Aides. We also have Honeywell Telemonitors and MedPartners available to patients who need more assistance monitoring vital signs and reminders for medications.Our company also uses Allscripts Homecare (EMR) Electronic Medical Records, making us THE MOST ADVANCED Home Health Care in Northern Indiana. Office hours are Monday through Friday 8:00am to 4:30pm and we have a nurse on call 24 hours a day, 7 days a week, 365 days a year. We are a Medicare agency that also accepts Private Insurance in addition to Medicare. We serve 8 counties in Indiana and 3 in Michigan.
