Clinical pathways in the NHS requires multiple patient appointments/phone calls to collect clinical data needed for diagnosis and management. This is a very manual process taking up significant amount of clinical time and resource and creates bottlenecks leading to delays in treatment. Remcare allows remote collection of clinical data and patient communication using a patient based smart phone app and a secure UK based cloud server. Algorithms on the Remcare platform analyse the risk profile of patients at every step of the care journey. Patients crossing pre set clinical thresholds are highlighted on the intuitive cloud platform and notified to clinicians by text/email messages thus allowing them to intervene early while minimising the impact of unnecessary appointments for low risk patients. This allows redesign of patient pathways to include fewer appointments, promote patient self- management and allow timely intervention (due to real time delivery of information) while decreasing the admin and human resource costs and improving productivity in the NHS. Remcare offers a template that can be used to create customised solutions for multiple clinical specialities and in primary and secondary care. It offers one platform for the whole of the patient journey and minimises need for multiple digital products for healthcare providers. Remcare is founded and designed by Sandeep Konduru who is a surgeon practising in the NHS.
