As an area advocate for quality hospice care, Rock River Hospice & Home (RRHH) has been providing expert end of life care for patients in the final months of life – and support for their families – for over 38 years. Putting the needs of our patients and their families first … that is our commitment to you. Our specialized services are focused on patient and family care and includes symptom management, emotional support, spiritual support and psychosocial services. We provide a multi-disciplinary approach which includes your personal physician, nurses, aides, social workers, bereavement counselors, and volunteers who form a team to address and care for the patients’ needs while focusing on the individual’s quality of life. Hospice services can be provided to patients in their own home, retirement center, long-term care facility, the hospice home or wherever the patient calls home.
