About Smart Rehabilitation

Smart Rehabilitation is a community-based service operating in the Yorkshire, Humber, Derbyshire, Nottinghamshire, and East Midlands. It has been brought together by a collaborative partnership between clinical specialists who are experts in their fields (NHS and private), have worked as expert witnesses in their specialist area, and have had years of experience treating clients within the medico-legal framework. We provide a solution-focused bespoke one-stop-shop MDT service that embraces the biopsychosocial model of healthcare and collaboratively delivers holistic rehabilitation based on the ICF framework to order to enhance the engagement and participation of our clients. At Smart Rehabilitation, we work across a vast continuum of healthcare environments including hospitals, nursing homes, gyms, hydrotherapy pools, clinics, workplaces, assisted living homes. We believe in delivering joint up care in the community, which is seamless, communicative, and person-centered. We foster trust, build relationships, and strive for excellent rehabilitation. We provide a team of leading experts from a range of specialisms to work together as a multidisciplinary team (MDT), centered around the patient’s rehabilitation needs. Your client’s personalized team may include rehabilitation consultants, specialist physiotherapists, neuropsychologists, speech and language therapists, occupational therapists, pain management specialists, CBT specialists, orthotist, and more. We provide treatment around multiple pathologies ranging from Brain injuries, spinal cord injuries, and complex trauma orthopedic to neurodevelopmental conditions, progressive neuromuscular disorders, musculoskeletal conditions and rheumatology, elective and acute orthopedics, burns, plastic surgery, oncology, a wide range of respiratory conditions including cystic fibrosis and COPD and long COVID.
