About Solace and Sage
A 501(c)3 nonprofit, created from the ReInventing Independence Initiative, dedicated to providing the structure and support for helping adults 60 years and older in Central Texas preserve dignity and quality of life. Our team of occupational therapy practitioners and nurses offer consultative and medical services in the comfort of one’s home that identify and address personal, medical and environmental needs for returning or remaining at home and aging in place. Our programs: Next Steps Program: Challenges managing and navigating unexpected circumstances with clarity and certainty; especially when facing a medical crisis or chronic and/or progressive illness at home. We provide the necessary structure and support to help organize thoughts, emotions and actions for focus and direction when determining next steps. We provide guidance along the path to feeling clear, confident and comfortable when making difficult decisions around personal, medical and environmental needs. Return Home Program: Gaps between the pre- and post-discharge process causing unnecessary stress, strain and risk of re-hospitalization. We provide the necessary structure and support for managing and navigating a safe return home through identifying and addressing your personal, medical and environmental needs. These provisions close the gaps between the pre- and post-discharge process. SAFE at Home Program: We assess the environment and make personalized recommendations for modifications that match limitations and abilities for remaining at home and participating in what matters most with peace of mind and confidence. Mobile Outpatient Occupational Therapy: We offer services in the comfort of home for preserving quality of life and improving health management of a new diagnosis or chronic and/or progressive illness. We provide personal, practical, and sustainable solutions for you and/or your caregiver for developing, managing and maintaining health and wellness routines.
