We have been serving Nassau, Suffolk and Westchester counties since 1986. Our mission is to ensure and maintain patient choice, dignity and well being. South Shore Home Health Services, Inc. is a New York State licensed home care agency dedicated to serving patients in their home environment. Our programs include Falls Intervention, Nursing Home Transition and Diversion Waiver, TeleHealth, and UTI Intervention. Our services are diverse - Physical and Occupational Therapy, Home Healthcare Aides, Infusion Therapy, Medication Management, Skilled Nursing Shifts, Skilled Nursing Visits, Telehealth and Family Support. Our focus remains on educating our patients and their families and providing compassionate care. South Shore Home Health Services, Inc. has been an active member of the New York State Association of Health Care Providers since 1988 and Joint Commission Accredited since 1996.
