Sunset Lotus is a holistic, trauma-informed provider specializing in Coordinated Family Support (CFS), full spectrum End-of-Life (EOL) services, & consultation for Title 17-licensed homes & Residential Care Facilities for the Elderly (RCFEs). Our role includes navigating complex care systems, connecting families with generic resources, & ensuring continuity across Regional Center supports so that families feel grounded & secure in both the present and the future. Our end-of-life work spans the full spectrum, offering advance care planning, grief & bereavement support, future care mapping, Circle of Support coaching, & caregiver trainings. We provide presence, structure, & clarity at each stage of decline, death, & aftercare, ensuring families are not left to carry the burden alone. We also extend our expertise to Title 17-licensed homes & RCFEs, consulting with administrators, service providers, & staff to integrate end-of-life planning into daily care, strengthen team communication, & address service gaps.
